LAS VEGAS — After successfully launching an Accountable Care arrangement and population health management partnership last fall, Humana Inc. (NYSE: HUM) and HealthCare Partners Nevada, a division of DaVita HealthCare Partners Inc. (NYSE: DVA), will further solidify the partnership by opening two new primary care practices in Nevada.
Opening under the HealthCare Partners Nevada brand, the two new primary care practices located in Southern Nevada are designed with Humana’s Medicare Advantage members in mind. The new practices are located at 6210 N. Durango Drive, Building 11 in Las Vegas and 745 S. Green Valley Parkway, Suite 160 in Henderson.
The practices will open just in time for the upcoming Medicare open enrollment period that goes from Oct. 15 through Dec. 7 and is when those eligible for Medicare are able to change their health plans and prescription drug coverage for the following year. For many, this is the only time of year these changes can be made.
Last year, HealthCare Partners Nevada, in partnership with Humana, opened three primary care practices in Las Vegas. This was an expansion of Humana’s Integrated Care Delivery Model with DaVita HealthCare Partners.
All HealthCare Partners locations practice the organization’s Total Care Model, an approach to health care in which primary care providers, specialists and professional staff work as a coordinated team to manage all aspects of patients’ care and overall health. This includes a strong focus on prevention and assisting patients with maximizing their health insurance benefits.
